I want this information for research. Answers provided would be used for statistical purposes and no direct name would be disclosed except pronouns and fictitious names if needed. If a name is used, then it would be done only after consent of the respondent. The research is done at personal initiative and would continue if the response is representative. Otherwise it will be discarded.
